Does it now comes with the thermal pad and rubber band included?

Only a rubber band. I used a thermal pad that came with my motherboard. I recommend ordering or buying a sticky thermal pad for it if your application does not have a cooling solution in place already, eg: a heatsink/fan combination that covers the M2 drive. thermal adhesive would work also as the heatsink is very well machined.

Does this come with any mounting hardware? what is the recommended mounting method? thermal conductive glue or adhesive tape?

It comes with rubber bands to attach to the M.2 drive. I recommend putting thermal paste on the drive as well before installing.

What is total thickness with thermal pad? 0.5?

I don't bother to open up my desktop to check the thickness. Generally speaking, you don't need to worry about the thickness if use it to a desktop PC. It may be too thick for laptops. This heatsink gives the most temperature reduction in comparing with all other kind of heatsinks that I used.
