Do I need to buy the methanol separately? What kind of water do I need to use?

Yes you need methanol for freeeze protection, it can be purchased at qwixmix.com. 
You can use regular tap water.

What do I have to use for freeze protection? I don’t understand the dosage in the back.

I feel the manufacture’s listing information on Amazon is extremely misleading. At least to me it gives you the impression that this product is a complete concentrate and all you need to do is add water. Wrong! 
That is for windshield fluid for summer use, and is misleading if you intend to use it during the winter. 
They don’t seem to mention (unless I missed the fine print) that you have to also buy methanol and that the price of the finished product is going to be significantly much more expensive if you intend to use it during the winter with antifreeze protection. 
They claim it makes 32 gallons which I guess it would for summer use, but if you want anti-freeze protection you’ve got to buy and add methanol (which even on Amazon the price is $12 - $99/gal) and to get -25°F protection which is typically what most windshield solvent antifreeze protects to at the price of $12 a gallon for methanol you have to add 48 oz of methanol (another $3 @ $12/gal) to get the same protection. 
So in reality this product is MUCH more expensive than a standard bottle of winter protection windshield washer solvent if you were to buy it somewhere other than a truck stop.
for this reason, I will never buy this product again. 
According to the manufacturer it only contains soap, water and coloring. Pretty expensive dish soap if you ask me.
You could probably do the same with a small amount of dishwashing liquid For a tiny fraction of what this costs.
… and besides, it doesn’t even smell good.

What is the technique of opening the cap? Pushing down and turning to the right doesn't work.

Try turning it counter clockwise.  I don’t remember it having a reverse thread.

There is no alcohol. You can use the chart on the bottle to add alcohol for freeze protection.
