Should I buy a 20×20 for an 18×18 cover or do these fit as expected?

Definitely buy the pillow bigger than the cover. So a 20 x 20 would be very well for an 18 x 18 if you want your pillow to stay poofy. Mine works very well doing that way.

Do they go flat over time?

We have only had ours for a couple of weeks but they’re very well-made and very firm so my guess is they will not go flat
