Can I connect this directly to wall current? The transformer looks so small that it makes me nervous

I have 6 of these ( I use the 5 VDC and 6 VDC output models) connecting directly to 120 VAC and potted into older security cameras as these are way cheaper than the batteries. Of course you are responsible for the safety of wiring and packaging as it has open terminals and exposed circuits that will be live AC when connected. It is not designed to be used as is without some type of enclosure which will be up to the user. These are tiny, they fit in the battery compartment of a popular wireless camera that uses 4 CR sized batteries.

Does polarity matter on the ac input?

Since it is AC, it shouldn't matter, unless for some reason you're attempting to do something on the DC side that compares to phase on the AC side (which I hope you're not doing).

Will this power a Raspberry Pi Zero W via the GPIO 5V and ground pins (is power smoothing and conditioning adequate)?

As long as the current rating is adequate enough. It should be OK. The switching noises on these are not so bad.
