As far as I can tell it does kill spiores but you have to stay on top of the problem.

Does the sprayer spray in a narrow stream or wider mist-like spray?

Both. You adjust the little knob on the sprayer. This stuff is very good.

Is this a enzyme cleaner/disinfectant?

Not really sure, I do mix 1/3 to 1/4 into laundry detergent and have had no problems with it.

Is this safe to spray a mattress?

Jason, the product is a disinfectant when used on hard non-porous surfaces. There is no specific guidelines for use on mattresses.
