This is for my home. I don't want a heavy hose, so does the metal head have a significant advantage over a plastic one?

I think the steam is better but it is so heavy. I would not buy a metal head again extra steam not worth extra cost and the weight.

how well does it work on wrinkled cotton shirts

Depends on how wrinkled it is. It works well though and gets the basic wrinkles out well. If it's been tied in a knot for months you may have to get the iron out. But I am in the clothing business and we steam all the wrinkles out with no problem
