Does this have more than one speed?

I can't really answer that properly. the computer uses the combo switch i believe to tell it what to do. when I installed it on my 1998 dodge ram 1500 pick up it had multiple speeds. It should be capable

Mine didn't come with the worm gear. What kind of worm gear should I get?

not sure what you mean by worm gear. mine was bolt together and run. nothing else was needed for my 98 1500

I am using this for a Halloween prop, what does each color wire do? I have yellow, white, red, green, and then black is ground.

Not sure how fast you want it to go but the black is ground, the red is power, the blue and yellow are 2 different speeds hi and low, the other is for the switch if you use it in a car

Does the shaft turn continuously in one direction or turn stop reverse turn?

yes the shaft turns continuously one direction unless you reverse polarity it will turn continuously reverse direction
