No, it does not come with an egg cup.

Does this device only work when an egg is placed in a regular egg cup? I can't get it to work properly when holding egg in my hand,

We (our family) call this wonderful device "the Thumper." If you think about it, lifting the rod on top and releasing it, uses weight and downward gravity motion to cleanly cut the top of the eggshell. Holding the egg in your hand, not on a firm surface, defeats this endeavor. : -
