I bought this under the impression that it came with the ps4 digital code but only got the ps 5 disc

If you buy a digital version you get both if you buy a disc, it's a disc version only.

Do i have to have an online ps membership to play this? some of the reviews leave me confused.

Your PS5 has to be connected to the internet to play this game, there is no offline play and the game will not load without a connection.
