It could work, but the design which causes the ball to pop out when put under pressure, makes it impractical to use for this purpose.

Does it roll in all directions, or just one way?

It rolls back and forward , it you want to change directions you mush turn at handle to change directions. ( Foward to 45 or 90 degrees, / up down to sideways)

Is the roller ball hard? Does the roller ball retain its shape as you massage with it? I wondered because the description mentions "gel technology".

The rollerball is a hard stainless steel ball with a handle that attaches on two sides of the ball. It is probably filled with a gel that freezes. It keeps the cold well and does not warm up while you were treating your injury. It really is an excellent product.
