Does the bluetooth work as a replacement for a usb transmitter. Meaning I can connect my bluetooth earphones to this and play soun

The Topping DX3 Pro+ can only receive Bluetooth from an external device like a smart phone. It does not have the capability to connect to Bluetooth output devices like a Bluetooth speaker.

Is the volume for the line-out fixed or variable? And can you listen on heaphone out and line out at the same time?

Headphone and Line Out are switched from the remote, only one of them can be active. Unless there is a mode I have not discovered yet.

What is included in the box? Any cables?

In the box: The DAC, wall wart for power, the antenna, the remote and a USB cable! So yes, a USB cable!
